Notice of Availability of IRS Form 1095-C

For the 2025 Calendar Year (Coverage Year) 

Under Internal Revenue Service (IRS) regulations, Catskill Central School District is required to make available Form 1095-C, “Employer-Provided Health Insurance Offer and Coverage,” to employees who were full-time for one or more months during the 2025 calendar year. 

Form 1095-C provides information about the health coverage that was offered to you, if any, by Catskill Central School District. This form may help you determine your eligibility for the premium tax credit when you file your federal income tax return. 

How to Request Your Form 1095-C 

If you would like to receive a copy of your 2025 Form 1095-C, you may request one using any of the following methods: 

  • By Email: payroll@catskillcsd.org
  • By Mail: Catskill Central School District, Attn: Benefits Department, 347 W. Main Street, Catskill, NY 12414
  • By Phone: 518-943-4550 ext. 1418 

Upon receiving your request, Catskill Central School District will provide your Form 1095-C within 30 days of the request date, or by January 31, 2026, whichever is later. 

Additional Information 

  • You do not need this form to file your taxes; however, you may keep it for your records. 
  • If you obtained coverage through the Health Insurance Marketplace, you will also receive Form 1095-A directly from the Marketplace. 
  • This notice will remain available on this website through October 15, 2026, in accordance with IRS requirements.
